We are currently seeking a Social Services Coordinator (SSC) to service properties in the San Jose area. The SSC will assist residents in improving the quality of their lives by connecting residents with community services; examine the need for on-site services and explore ways in which these programs and services can be brought to the community; link residents within the community to supportive services and/or medical services provided by public agencies or private practitioners within the general community. In addition, the SSC will monitor the ongoing provision of services from community agencies and keep the case management and provider agency current with the progress of the residents. Majority of time is spent in the property offices with subsantial movement throughout the properties, neighborhoods and cities Some driving is required. We offer competitive pay and benefits.

Job Requirements:
Candidates must have 2+ years of social services experience including working with seniors, disabled and families. Experience in long and short term interventions and case management and experience working with the elderly and disabled is preferred. Excellent written and verbal communication skills. Must have a valid CA driver's license.
