REGISTERED PHYSICAL THERAPIST
The Center for Elders Independence is a non profit organization serving frail elders in the East Bay. Our innovative, all-inclusive, long-term health care program (PACE Program) provides the frail elderly with all the care they need, allowing them to maximize their health & independence and remain close to their family members and within the community.

We are actively seeking a compassionate, experienced PT to join CEI’s interdisciplinary team! Much less paperwork is required for this PACE model of care.

POSITION: Under the administrative supervision of the Site Manager and the technical instruction of the Rehabilitation Coordinator, The Physical Therapist provides restorative and rehabilitative services in accordance with they physician’s written authorization and in conjunction with the interdisciplinary plan of care.

DUTIES:
(Primary duties and responsibilities – may not include all of the duties assigned)
1. Evaluates agency participants by applying muscle, nerve, joint and functional ability tests.
2. Provides assistance to achieve and maintain maximum performance using physical means such as exercise, massage, heat, sound, water, light or electricity.
3. Provides active and passive exercises, muscle re-education and gait and functional training activities of daily living including transfer activities and prosthetic training.
4. Instruct participants and caregivers in the care and use of wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.
5. Instructs inter-disciplinary team and family on participant’s total physical therapy program and phases of physical therapy with which they may work with patient.
6. When appropriate, and with written physician’s authorization and /or team advise, treats participants to relieve pain, develop or restore function and maintain maximum performance, using physical means, such as exercise.
7. Conducts assessments on all participants every six months
8. Observes, records and reports regularly to the interdisciplinary team on participant’s reaction to physical therapy program and any changes in the participant’s condition.
9. Instructs and supervises therapy program aides in the implementation of the participant’s individualized rehab plan of care.
10. Provides in services training to other members of the CEI staff.
11. Attends daily interdisciplinary team meeting and actively participates in the development of the plan of care.
12. Evaluates the home environments and makes appropriate recommendations as needed.

QUALIFICATIONS: Requires a bachelor’s degree and a graduate of an accredited school of physical therapy. Currently licensed as a Registered Physical Therapist. At least one year’s experience working as a Registered Physical Therapist with a frail or elderly population.
