Responsibilities
• Admit patients to the services, completing appropriate clinical documentation under the Clinical Manager's direction. Assess individual status and care/instruction needed.
• Administer on-going care and case management for each patient, and provide necessary follow-up as directed by the Clinical Manager.
• Confer with physician and develop the initial Plan of Treatment based on physician's orders and initial patient assessment.
• Provide hands-on care, management, and evaluation of the care plan.
• Partner with Clinical Manager to educate patient in accordance with physician orders. Revise plan in consultation with physician based on ongoing assessments and as required by policy/regulation.
• Coordinate appropriate care, encompassing various healthcare personnel - to include Physical Therapists, Occupational Therapists, Home Health Aides, and external providers.
• Analyze any changes in condition, and identify symptoms and severity; and continuously report patient care/condition/progress to patient's physician and Clinical Manager.
Qualifications
• Diploma; or Associate's or Bachelor's in Nursing.
• At least 6 months' experience in area of specialty/desired practice.
• Current unrestricted licensure to practice as a Registered Nurse in the state(s) to which assigned and current CPR certification.
• Good organizational and communication skills.
• Ability to work autonomously.
