JOB DESCRIPTION:
Population Health and Prevention’s Housing and Urban Health is accepting applications for a par-time position (up to 39 hours/week) in class 2320 Registered Nurse [Public Health Program] for the Action Point Program. The Action Point Program’s client base includes indigent, uninsured, homeless or marginally housed HIV+ individuals who may also suffer from mental health and/or substance abuse conditions. The incumbent will serve as the lead nurse for the program. Action Point is located within the Housing and Urban Health Clinic in the Tenderloin neighborhood of San Francisco.

New employees hired for this position will be covered by the Local 790 Registered Nurse collective bargaining agreement (CBA) and shall receive a $5,000 bonus payable as follows: $1250 after 6 months of employment, $1250 after one year of employment, $1250 after eighteen months of employment and $1250 after twenty-four months of employment.

Appointment Type: Permanent and Provisional applicants encouraged to apply. Provisional applicants must participate in a Civil Service examination for this job classification to become permanent.

ESSENTIAL DUTIES:
Under the general supervision of the Nurse Manager and lead physician for Action Point, the Registered Nurse works at the Action Point Center and provides both nursing and specialized adherence support services to clients; works as part of a multi-disciplinary health care team; provides direct adherence support services and occasional general urgent care services as part of the HUH Clinic staff. The essential job functions include the following:
• Coordinates clinical activities including nursing case management that ensure quality client/patient care for a cohort of HIV+ homeless/marginally housed individuals;
• Demonstrates problem-solving skills;
• Provides client/patient care in a manner that is both non-discriminatory and non-judgmental;
• Identifies issues and problems and bring to the attention of supervisor;
• Performs initial and continual assessment of client’s health status, immunizations, TB screening and dispensing of medications, based on established protocols;
• Communicates effectively and collaborates with members of the inter-disciplinary team and members of the community;
• Assesses and treats clients/patients who may have substance abuse problems, homelessness and mental health issues;
• Performs all other requirements and duties as listed in job duties of a 2320.

MINIMUM QUALIFICATIONS:
1. Possession of a valid permanent/temporary California License as a Registered Nurse. A photocopy of license must be submitted with the application when filing; AND
2. One year of Registered Nurse experience in an acute care setting, ambulatory care clinic, primary care clinic, public health setting or Home Health.

Verification/Waiver: Verification of qualifying experience, education, and/or training is required at the time of filing. Candidates unable to do so may submit a letter requesting a waiver of this requirement indicating the reason(s) a verification cannot be obtained. Failure to submit verification or request for waiver will result in application rejection.

Condition of Employment: Possession of a valid Cardiopulmonary Resuscitation (CPR) Certificate is required as a condition of employment. Appointees will be required to obtain the certificate within two weeks of hire date. A photocopy of the valid Cardiopulmonary Resuscitation (CPR) Certificate must be provided to the supervisor during this period. Failure to show proof of certification within two weeks of hire date may result in the dismissal and/or termination of employment.

DESIRED QUALIFICATIONS:
• One year experience working with the homeless population.
• One year experience working with people with HIV/AIDS.
• Evidence of recent experience working with HIV clients and expertise in adherence and treatment strategies.
• Possession of a BSN degree.
• Possession of a BLC-S certificate.
• Extensive HIV disease and anti-HIV medication knowledge.
• Spanish language fluency
• Commitment to working with the urban poor and social justice.
