JOB DESCRIPTION:
Population Health and Prevention’s Children’s Medical Services, Child Health & Disability Prevention Program (CHDP), is accepting applications for two (2) full-time positions in class 2830 Public Health Nurse for the CHDP Foster Care Unit. Candidates may be considered for other positions, as they become available, within the next three (3) months.

New employees hired for this position will be covered by the Local 790 Registered Nurse collective bargaining agreement (CBA) and shall receive a $5,000 bonus payable as follows: $1250 after 6 months of employment, $1250 after one year of employment, $1250 after eighteen months of employment and $1250 after twenty-four months of employment.

Appointment Type: Permanent and Provisional applicants encouraged to apply. Provisional applicants must participate in a Civil Service examination for this job classification to become permanent.

ESSENTIAL DUTIES:
The Public Health Nurse provides coordination of outreach, provider training and quality monitoring of child health services, review of case management activities and referral for follow-up and treatment of diagnosed conditions for children in the county or in foster care. The essential job functions include the following:
• Informing eligible CHDP individuals or groups, of the components of a well examination and diagnosis and treatment services; making home visits to foster parents;
• Identifying medically at risk children and facilitating timely referral for medical and dental care services, interfacing with hospital discharge planners, providing health consultation services to Child Welfare Workers;
• Interpreting suspected conditions to clients and assisting and arranging diagnosis and treatment services; providing high risk case coordination services to foster children in high risk programs;
• Collaborating/conferring with primary care provider, Child Welfare Workers and specialists to facilitate client access to diagnosis and treatment services;
• Training and supervising paraprofessional staff to determine child/family need for preventative medical and dental services;
• Conducting review of completed health assessment forms to assess provider’s compliance, trends and a need for assistance to comply with CHDP medical guidelines;
• Participating in Outreach, Education and Quality Improvement activities.
• Performing related duties as required.

MINIMUM QUALIFICATIONS:
1. Possession of a valid permanent/temporary California License as a Registered Nurse (a photocopy of license must be submitted when filing); AND
2. Possession of a Public Health Nurse Certificate issued by the California State Department of Health; AND
3. One (1) year of verifiable experience (within the last five years) as a Registered Nurse in an Acute Care Hospital, Primary Care Facility, Home Health Agency, and/or Public Health Agency within the last five (5) years.

Verification/Waiver: Verification of qualifying experience, education, licensure, certification and/or training is required at the time of filing. Candidates unable to do so may submit a letter requesting a waiver of this requirement indicating the reason(s) verification cannot be obtained. Failure to submit verification or request for waiver will result in application rejection.

Condition of Employment: Possession of a valid Cardiopulmonary Resuscitation (CPR) Certificate is required for a condition of employment for all position. A photocopy of the valid Cardiopulmonary Resuscitation (CPR) Certificate must be provided to the supervisor within two (2) weeks of hire. Failure to show proof of certification within two (2) weeks of hire date may result in the dismissal and/or termination of employment.

DESIRED QUALIFICATIONS:
• One (1) year of pediatric nursing experience.
• Possession of a valid/current California Driver’s License
• Two (2) years of public health nursing in a community health setting (e.g. Outpatient Primary Care Facility, Home Health Agency, or Public Health Agency).
