Hospice and Palliative Care of Contra Costa is a community based program for terminally ill patients. Care is given to patients and families by an interdisciplinary group consisting of physicians, registered nurses, home health aides, social workers, chaplain, dietician, pharmacist and patient support volunteers. Bereavement care is provided to the family for one year after the death. Hospice services are given regardless of the ability to pay, making the agency dependent on donations and insurance billing to meet operating expenses.
Reporting to the Team Manager, this position is responsible for phone advice, scheduling staff as appropriate, identifying and coordinating patient/family care needs under the direction of the attending physician and/or Medical Director and other members of the interdisciplinary team. This position is also responsible for the hiring and management of a team of nurses that are available to provide Continuous Care to Hospice Benefit patients.
POSITION REQUIREMENTS
Demonstrates excellent interpersonal and communication skills, initiative, dependability and good judgment.
Knowledge and understanding of hospice philosophy and hospice criteria for services under the Hospice Benefit.
Knowledge of State, Federal regulations and JCAHO standards
Demonstrates excellent skills in standards of practice for hospice care including pain and symptom management
Must be able to work well under pressure meeting multiple priorities and sometimes competing deadlines
Demonstrates strong organizational, time management and development skills
Demonstrates, at all times, cooperative behavior with colleagues and management staff
Ability to assess and respond to needs of patients, families and caregivers via phone communication.
Ability to communicate effectively with family and assist with coping and emotional stress; is tolerant of individual lifestyles and is nonjudgmental.
Demonstrates ability to work collaboratively as part of an interdisciplinary team as well as independently, and problem solve in a crisis situations.
EXPERIENCE AND EDUCATION
Current California Registered Nurse License required.
At least two years experience working as a professional nurse caring for the terminally ill
Current California Drivers license
CPR certification required
BSN or MSN preferred
At least two years prior supervisory or management experience preferred
Certification as Hospice and Palliative Care Nurse preferred
IV skills highly desirable
