MAJOR RESPONSIBILITES:
Residents
Always greet residents and guests with their name and a friendly smile.
Quality Assurance of all client care (i.e. assure that ADL schedules are being followed). Make sure residents have clean clothes and hygiene at all times.
Ensure that employees are communicating with residents in a respectful and supportive manner.
Conduct a weekly audit of all documents including ( but not limited to) 1) resident roster 2) face sheet 3) medication summary 4) PRN record 5) medication refusal form 6) ADL sheet 7) vital signs
8) kardex file 9) centrally stored medication record 10) medication order forms 11) accuracy of medication bottle labels, start and expire dates
(If you pass medications - review Medication Summary and record 1) Medication Refusal and 2) PRN Record.)
Ensure that medication protocol is being followed and medications are being set-up correctly.
Coordinate medication management meetings.
Read daily log notes - every morning, giving special attention to residents who needs help ASAP.
Daily update of communication log. Be specific.
Ensure that the residents are scheduled to see their Podiatrist every three months & Dentist twice a year and recorded in the system.
Conduct a daily facility walkthrough where you greet, listen to and observe the well being of each resident and staff member.
Ensure that special instructions regarding residents health conditions are being followed.
Create/ update care plan of all residents every month
Work with Assistant Administrator in submitting special licensing forms such as waivers and incident reports to Licensing.
Ensure that the residents, facility, and staff are meeting licensing regulations.
Maintain regular contact with families, social workers, case managers, nurses, physicians and responsible parties by keeping them informed about their loved ones/clients in a positive manner.
Assist residents with walking and activities if needed
Report all unusual incidents including suspected abuse.
Other
Assist with the development of policies and procedures.
Assist with scheduling medical appointments as needed.
Answer telephones and direct calls.
Maintain lobby, nurses station and personal work area clean, organized, and free of clutter.
Other duties as assigned.
Facility
Maintain a clean and sanitized facility at all times.
Notify and coordinate facility repairs with Facility Coordinator
Ensure Community Care Licensing and OSHA compliance such as a complete MSD book and posting of work-related accident form.
Activities
Provide assistance to Activities Coordinator so as to ensure the implementation of a creative structured activities program that involves all residents, employees, Pacific Institute interns, and volunteers.
MIMIMUM QUALIFICATIONS AND KEY COMPETENCIES:
Must have genuine love and concern for elders.
Acceptable Health Screening report.
TB clearance.
Fingerprinting clearance from DOJ.
Must be at least 18 y/o and eligible to work under local, state, and federal laws.
Strong computer, organizational and writing skills.
Customer Service Experience.
Knowledge of and ability to operate and maintain equipment: FAX machine, calculator, photocopier, telephone, computer and printer.
Reading, comprehension and writing ability.
Licensed as a California driver with a clear record.
