QUALIFICATIONS:
1. Bachelor’s Degree in Psychology, Sociology, related field, or at least two (2) years of experience working with clients who have a serious and persistent mental illness
2. Experience in case management and in coordinating services for clients.
3. Excellent interpersonal and client engagement skills and experience.
4. Knowledge and understanding of records charting.
5. Excellent communication skills, verbal and written.
6. Ability to work with minimal supervision.
7. Ability to work well with physicians, nurses and other clinicians. Must work as a team player.
8. Ability to handle multiple demands and /or crisis situations in a professional manner.
GENERAL REPONSIBILITIES: The Mental Health Specialist is responsible for
coordinating client intake and admission as well as coordinating program and case management services with other services providers. The Mental Health Specialist also provides daily support to assigned physician.
RESPONSIBLE TO: Program Administrator
SPECIFIC RESPONSIBLITIES:
The Mental Health Specialist shall:
* Support MD ACFE
1. Provide daily support to assigned Psychiatrist.
2. Complete initial intake/admission paperwork on all new clients. Develop a New Client Orientation Packet and ensure that at a minimum every client receives: Patient Rights Forms; New Clinic Packet; Clinic Policies/Procedures; After-Hour Telephone #s, etc.
3. Complete initial and annual ACFE’s (Annual Community Functioning Evaluation) on all clients within the program.
4. Coordinate with the Program Assistant and Billing Clerk to ensure that eligibility verification has been completed on each client seen each day.
5. Develop engagement strategies with all clients enrolled in the clinic (i.e. brokerage, linkage, milieu management).
6. Ensure that the waiting time for clients in the lobby is reasonable at all times in an effort to support each customer returning for services.
7. Manage the flow of clients for his/her assigned Psychiatrist for that day.
8. Document daily contacts with others involved in the client’s care (i.e. case managers, ACCESS, family members, residential care providers, guardians, hospitals discharge planners, etc.)
9. For all new clients, monitor client participation in treatment and design a client engagement system for immediate follow up in the event a client misses a scheduled appointment.
10. Assist in obtaining insurance cards; processing TARs; maintaining housing; and providing a list of resources for clients in need of these services. If working with children/adolescents, this also includes making contact with schools, courts, family services, etc. as needed.
11. On a daily basis, check each chart of clients seen that day to ensure that physicians, clinicians and clients have signed and completed all pertinent forms (medical consent forms; client treatment plan, etc.)
12. Work closely with Psychiatrists, Nurses, and/or Therapists on discharge planning for assigned client.
13. Complete required records documentation daily/weekly /annually and keep
medical records and clinical documentation current at all times. All notes must be legible.
14. Attend staff meetings and communicate clinical concerns to team.
15. Participate in Continuous Quality Improvement Management team programs as
requested by the Program Administrator. (i.e. distribute client satisfaction survey to each client at each visit to the clinic, monitor how many clients are being hospitalized each month, etc.)
16. Participate in special/emergency cases, case conferences, and client staffings as required.
17. Provide all clients with care, dignity and respect -- continuously striving to deliver the highest level of support and client-centered treatment.
18. Perform other duties as assigned by the Program Administrator.
19. Deliver 85% direct client services.
