Job Title: San Francisco Community Clinic HealthCorps Member
Payroll: Education Award - $1250.00
Commitment: 450 service hours beginning March 2006 (and ending no later than August 31, 2006)

Ocean Park Health Center (OPHC) is a primary care clinic affiliated with the San Francisco Department of Public Health (SFDPH). It is located in the Sunset District of San Francisco and serves the Sunset, Richmond and Outer Mission neighborhoods. OPHC provides primary care services, social work support, health education and nutrition counseling. OPHC has a patient panel of 5,100 patients, the majority of whom are low-income and uninsured or on public insurance such as Medicaid. The patient population is multicultural and largely immigrant, with about 50% being Asian American, mostly monolingual Cantonese speaking, 30% Russian-speaking , and 6% Korean speaking patients. Approximately 40% of all the patients do not speak any English.
You will be involved in the diabetes medical care at OPHC in several ways. Firstly, you will meet with patients for individual Self Management sessions. During these visits patients learn problem-solving skills by being taught to develop small focused goals for long-term behavioral change that will improve their overall health and control of their diabetes.
You will also meet weekly with the clinical team and patients to examine ways to improve self-management strategies. You will coordinate a series of patient focus groups to find out directly from patients from different ethnic groups and cultures what the perceived needs are. You will devise and administer surveys to patients to measure if these strategies are effective. You will assist with compiling the data and analyzing it to determine how to focus clinic resources more productively in helping patients.
You will learn to use our Diabetes Registry CDEMS and work the registry to follow-up with patients who need more intensive treatment or have fallen out of care.
As with many chronic conditions, depression is a major barrier to improving health. You will work with the clinic team to hold patient support groups that focus on self-management and stress-management and teach them the problem solving tools necessary to live with a chronic illness. Your contribution to diabetes care here at OPHC will give you extensive experience in chronic disease management, knowledge of the barriers to care in our population, and an understanding of clinic operations. This is an invaluable learning opportunity for all involved.

The San Francisco Community HealthCorps is an AmeriCorps Program that prepares members for a career in community health. This is a position that is suitable for graduate students or individuals interested in the health field. This may satisfy your internship/practicum requirements - please contact your department. The educational award will be awarded after completion of 900/450 hours and it can be applied to tuition and costs of attendance at any accredited educational institutions and federal loans.
