The Perinatal Council is seeking a Bilingual Family Advocate to provide case management to diverse clients that are pregnant and parenting adolescents in Contra Costa County. We use a home-visiting model, so a reliable vehicle with insurance is required. This position is based in Pittsburg.
The successful candidate understands grassroots community networking and enjoys reaching out and talking with people for the purpose of connecting them to services and community resources.
Specific duties include providing support and aid to individuals and families and coordinating activities to ensure clients and/or community members receive comprehensive care.
• Assist in the identification of eligible program clients
• Conduct individual intake interviews and program assessments
• Maintain active caseload of required families
• Perform mental health, substance abuse and other risk assessments
• Make referrals to appropriate services and follow up on care
• Maintain case notes per documentation requirements

Program Supervisor/Coordinator
QUALIFICATIONS
A key qualification is the desire to identify and help people who want improve their individual or family situation. In addition, the successful candidate must have related knowledge of the community, understanding of adolescent growth and development including the knowledge of parenting, infant and child development. Must have the ability to communicate effectively with families.
EDUCATION and/or EXPERIENCE
In-depth experience (3-5 years) working with adolescents in community outreach activities, church programs, or other program or case management experience may qualify as well.
Must have valid California Drivers License and car available for work; mileage reimbursement provided.
