SALARY: $32,000 - $36, 000 (DOE) + benefits (after 90 day introductory period)

The Center opened in 1996 as a nonprofit community resource center providing services around health & wellness, art & culture, computer training, senior wellness, and family support services. Summary

Under the direct supervision of the Clinical Supervisor, and the general supervision of the Program Director, the Family Advocate (Team Leader) may be assigned to one of two programs: Another Road to Safety (an innovative program serving low to moderate risk families who are diverted from Children's Protective Services (CPS) to community based, intensive family support services) and/or other PJCCE Family Support Services programs. The Family Advocate (Team Leader) is responsible for coordinating comprehensive services for clients in the West Oakland community and providing in-home support services to families that will be empowering to parents and children. Additionally, the Family Advocate (Lead) will serve as the team leader for the Family Advocates and will provide general guidance to the team. The Family Advocate (Team Leader) will be responsible for the following activities:

Responsibilities

- Conduct intake interviews with clients and make appropriate assessments of the clients and families.
- Serve as the team leader for the Family Advocates and provide general guidance on family support issues.
- Conduct intake interviews with clients and make appropriate assessments of the clients and families.
- Develop comprehensive service plans for clients and their family members with similar goals, service needs, and methods of intervention.
- Make referrals to appropriate services within and outside of the program as determined by various assessments.
- Maintain ongoing contact with clients, family, and other service providers in order to monitor service delivery and assess additional needs.
- Conduct at least 2- 3 weekly home visits with all family members participating in the program.
- Provide case management services to clients, and significant others, as appropriate to increase the client’s participation in the program.
- Establish familiarity with outside agencies regarding services they provide. Document all services and referral requests.
- Assist with the identification and development of training needs.
- Attend regular program/team meetings and all training workshops.
- Prepare all required data and reports in a timely manner.
- Develop, implement and monitor comprehensive case plans for clients.
- Keep accurate documentation of all activities for each client.
- Make appropriate referrals to community-based providers.
- Recognize emergency situations and immediately contact the proper persons.
- Perform other duties as required by the Clinical Supervisor and/or Program Director.

Minimum Qualifications

- BA degree from an accredited institution in psychology, human services, sociology or related field required. MSW highly desirable.
- Minimum 3 years experience as a case manager required, preferably with providing in-home family support services.
- Minimum 1 year experience serving as a team leader and/or supervisor.
- Knowledge of the resources in and around West Oakland required.
- Knowledge of MS Office Suite - especially MS Word, Access, Outlook, Excel, and Power Point required.
- Respect for and ability to work with people from diverse backgrounds.
- Possession of a valid CA Driver License, good driving record, and reliable transportation
- Ability to pass fingerprint check (e.g. LiveScan)
- Ability to communicate effectively in English
- Bilingual (English/Spanish) highly desirable.
