The Chronic Disease Case Manager coordinates the planning, implementation and evaluation of chronic disease management and education for health plan members. Identifies, screens and monitors potential high dollar and chronic cases and develops and works with the provider case manager coordinating the most medically appropriate, cost-effective plan of treatment. Responsible for assisting with coordination of the Disease Management process along the continuum of care and functions as a community resource and point of contact for members, health plan staff, providers and community agencies.

Requires: Current California RN license. Minimum 3 yearrs' clinical experience preferably working with chronic diseases in a community setting.
