Central Coast Alliance for Health (the Alliance) is a locally governed and publicly funded County Organized Health System serving over 85,000 members in Santa Cruz and Monterey Counties. Since 1996, the Alliance has worked to improve health care for people living in the Central Coast region.
Our programs include Medi-Cal Managed Care and Healthy Families in both Monterey and Santa Cruz Counties. Additionally, the Alliance administers the Healthy Kids Program in Santa Cruz County and the Alliance Care In Home Supportive Services (IHSS) Program in Monterey County.

The Chronic Disease Case Manager coordinates the planning, implementation and evaluation of chronic disease management and education for health plan members. Identifies, screens and monitors potential high dollar and chronic cases and develops and works with the provider case manager coordinating the most medically appropriate, cost-effective plan of treatment. Responsible for assisting with coordination of the Disease Management process along the continuum of care and functions as a community resource and point of contact for members, health plan staff, providers and community agencies.
Requires: Current California RN license. Minimum 3 yearrs' clinical experience preferably working with chronic diseases in a community setting.
